Forms Related to Pay and Leave, Timesheets

Automated SPA Timesheet (current through 2010)

SPA time sheets should be signed and dated by you and your supervisor. The completed form should be turned in to Edie Nowell in 3002 Engineering Building I (EB I) within five working days of the month following the pay period.

This form will calculate your totals, automatically fill in the dates for the current pay period and carry your personal information over from one month to the next. The form must be updated at the end of each year. The updated form will work in the same way as the old form.

A simple, non-automated, fillable version of this form can be found in Human Resources Forms, along with prescription drug forms, medical claim forms, salary classification forms, etc.

Acrobat Reader and most browser plug-ins will allow you to fill in the form but not to save it. This will prevent you from using many of the features of the timesheet. It is recommended that you download the timesheet and use it with Acrobat Standard or Acrobat Professional.

NOTE FOR NON-MSE EMPLOYEES: The pay period for SPA time sheets runs from the first Saturday of the month to the last Friday of the month. Some departments go by a timesheet schedule that goes from the first day of the month to the last day of the month. The SPA Timesheet for the first-day-to-last-day schedule is available here.

Biweekly Timesheet (generated using the Payday system)

We are now using the Payday system to complete Biweekly Timesheets. You should already have access to the system. If you do not, please see Meghan Johnston to be added.

You should enter your times into the Payday system after each work period. At the end of the period, "lock in" your times (see the Payday system for instructions on how to do this) and print the completed timesheet. Sign, and date your timesheet and get your supervisor to sign and date it. Turn in the completed form to Megan Johnston on payday.

This is an abbreviated version of the more detailed timesheet schedule that can be found at the human resources website.

EPA Leave Record

EPA Leave Forms are to be filled out and signed by your project's head (the P.I.) within the first five working days of each month. Forms are then to be turned in to Megan Johnston or faxed to her attention at 5-7724. You may also turn your form in to Edie Nowell or Jan Jackson.

Flat Rate Request Form

This form should be filled out and submitted to Human Resources by administrative personnel who need to pay an employee a lump sum.

Graduate Appointment Authorization

The Graduate Appointment Authorization should be used by people who know what to do with it. If you don't know what to do with it, you don't need to worry about it.

Request for Grader Pay for Graduate Students

This form should be filled out by the student, and then signed and dated by the person in charge of the grading and the faculty member who is authorizing the payment.

Request for Grader Pay for Undergraduate Students

This form should be filled out by the student, and then signed and dated by the person in charge of the grading and the faculty member who is authorizing the payment.

Request for Additional Pay for Graduate Students

Request for Additional Pay for Graduate Students should be used by graduate students who are requesting pay in addition to their regular salaries. The form should be filled out and signed signed by you and the head of your project (the P.I.). It should then be sent in by FAX to 5-7724 or turned in to Edie Nowell (EPAs) in 3012 Engineering Building I (EB I) (Campus Box 7907) or Edna Deas (grad students) in 3078 Engineering Building I (EB I).

Summer Salary Form

This form should be used by faculty who are requesting summer salary in addition to their 9-month salary. Please follow these instructions:

If you plan to charge your Summer Salary to C & G - 5 ledger accounts belonging to the MSE OUC range, please get in touch with the Contracts Manager to make sure that account has enough money to pay it. with the account(s) you plan to use. Once he approves the financial status of each particular project you may submit your signed form to him for approval. If your Summer Salary should be paid from other than C & G funds please submit your form to Edie. If your Summer Salary is budgeted from funds for projects belonging to other departments/colleges please contact the appropriate administrative staff in those departments for assistance in checking the availability of funds prior to completion of the Summer Salary form.

You should have either an email or the Summer Salary form with a signature from the administrative staff before submitting the form to Edie Nowell.
